The primary objective for the physician is to provide long-term patient-provider relationships, coordinate care across a spectrum of health services, educate, and offer disease prevention programs for ambulatory patients. This include stabilization and referral of those who require a different level of care. Physician will be responsible for the evaluation and treatment, including preventive care of a panel of patients assigned under their care and functions as part of a multidisciplinary team.
