There is a full time opening for a Physician in Primary Care at the Appleton Community-Based Outpatient Clinic within the Northeast Wisconsin Ambulatory Care Clinics. Responsibilities include a collaborative practice within our primary care clinics, with oversight of a designated panel of veteran patients. The primary care physician will be responsible for a panel of up to 1200 patients and a workload on average per day of up to 15 patients. The primary care physician is a member of a Patient Aligned Care Team (PharmD, dietitian, social worker, and Nursing) that is responsible for managing the health prevention/disease management needs of the assigned panel of patients, including urgent care. The incumbent will also be responsible to complete clinical reminders, achieve clinical performance measures, medication reconciliation, and be able to work within the computerized patient record system (CPRS). The focus of Primary Care at the Northeast Wisconsin Ambulatory Clinics is the provision of general medical care for veteran patients in a manner which is reflective of good clinical judgment, medical knowledge, clinical skills (medical interviewing, physical examination, and procedural skills), humanistic qualities, and professionalism.
