The Community Living Center Section Chief is responsible for providing oversight and leadership of our Community Living Center, Palliative Care/Hospice Unit and Geriatric Evaluation and Management (GEM) program. Physician will provide direct patient care, offer support to the provider staff and work with additional interdisciplinary staff. He/she will work closely with the Associate Chief of Staff (ACOS), and Associate Chief Nurse for GEC. CLINICAL RESPONSIBILITES During clinical time the incumbent will Provide medical oversight, leadership and supervision of a 120-bed Community Living Center (CLC). This includes supervision over the medical/provider staff who work in multi-disciplinary teams to coordinate, facilitate and provide patient care and services. Diagnose and treat medical and geriatric disorders using the most appropriate techniques in providing quality care. Provide supervision, as appropriate, of fellows, residents and students conducting direct clinical care. Deliver in-service training for medical center staff, residents, and medical students. Be responsible for timely completion of all clinical charting and documentation required by the Medical Center as appropriate and outcome data as appropriate. Perform transitional care as a consultant providing face-to-face consultation to patients under the care of the medical center's inpatient and emergency department services as appropriate. Educate Veterans, staff, and family about geriatric syndromes, functional assessment, and end-of-life issues. Consult with medical center staff on a wide variety of patient care issues. Remain involved in program evaluation. Maintain full clinical privilege as a member of the Medical Staff. Abide by Medical Staff By-Laws. ADMINISTRATIVE RESPONSIBILITIES Has direct administrative supervision of the Community Living Center physicians and nurse practitioners. Serves as a consultant to other medical center staff and trainees. Represents the CLC on decision-making committees at the Geriatric and Extended Care Service. Represents the CLC at professional/program community group/events and committees. Assists the ACOS of Geriatric and Extended Care on the coordination of Community Living Center services response in the event of critical occurrences of violence at the facility, disasters, and/or mass casualty occurrences. Is responsible for timely completion of all program evaluations and documents required by the Long-Term Care Institute, ensuring compliance and monitoring entities as appropriate. The incumbent has the supervisory responsibilities noted above, with additional responsibilities that include. Managements of Human Resource aspects of the Community Living Center relations, performance appraisals, and professional boarding procedures for direct employees. ii. Monitoring and reporting to the Geriatric Steering Committee regarding status of CLC Quality Measures, with implementation of needed action plans regarding QM and cooperative efforts to ensure the success of QM assigned to facility services. Decisions are made exercising wide latitude and independent judgment. The incumbent reports to the Associate Chief of Staff Geriatrics and Extended Care but operates on a day-to-day basis in an independent manner, with full responsibility to keep the Associate Chief of Staff Geriatrics and Extended Care apprised of activates and any issues in need of close collaboration.
